Abigail was in a mental facility for many, many months before Chad DiMera (Billy Flynn) was able to bring her back in her right mind.

Who knows what a direct injection of that drug, which Gwen had gotten from Dr. Wilhelm Rolf (William Utay) may have done? Sarah may not even know who she is let alone who Xander is, and she may even have brain damage as out of it as she was!
